Replacement Badge Request Form and Payment
REPLACEMENT BADGES - Use this form to request and pay for a replacement badge when your original badge is broken, or lost.  The cost for a replacement badge is $25.00

If your badge is Not Functioning but is still in good condition, send an email to ecclmembership@eriecountycl.org explaining the issue FIRST before completing this form.  Your badge may just be too old and there is no cost for the replacement.

If you have lost, broken or non-functioning guest badge or Junior badge, please send a detailed email to ecclmembership@eriecountycl.org.  No payment is required.


If your badge is lost or broken you are required to return it before the new one can be issued.  Send it to: ECCL, 815 Mason Rd E., Milan, OH 44846  The replacement badge will then be mailed to you unless other arrangements have been made with a Membership Committee member. 

Payment for the replacement badge is available following your submission of this form by credit card.  Note additional transaction fees may be applied.

Other Requirements - it is possible over time and upgrades to our badge system that we no longer have your photo on file.  Should that occur we will reach out to you and request an electronic photo be sent prior to issuing a new badge.  The photo should also go to the membership committee email address.
